Handsam has created a new model template for schools and academies to show good practice in supporting pupils in schools with medical conditions. This template can be adapted to fit local needs and the needs of individual pupils.
It can be used as an aid to schools in providing individual medical support for pupils. Schools and academies can adapt them as they see fit and may design their own template from this source.

What Does the New Template Include?
The new example template includes:
- Guidelines for the information you need about the pupil in question, such as:
- Contact details
- Prompts about their medical condition (dosage, administration, triggers, side effects, equipment, etc.)
- Space for you to consider emergency and evacuation information
- Prompts about school trips and off-site activity arrangements
- Space for family contact information and GP/hospital information to be added
- Consideration of staff training
- Space for parent/guardian agreement details and signature
Why might a pupil need a health care plan?
Health Care Plans are developed with joint input from the school, parents, the pupil and relevant healthcare professionals who can advise on the child’s case.
The goal of a health care plan is to support the child through the provision of knowledge and information about their medical condition, administration of medicines, what to do in an emergency and relevant contact information.
